NDIS Support Coordination Engagement Process​

Receive Support Coordination Funding

Your NDIS journey with Support Coordination begins when funding is included in your plan. This funding allows you to engage a Support Coordinator who can guide and assist you in navigating services, managing your plan, and achieving your goals effectively.

Contact Your Chosen Support Coordinator or Provider

Once you have funding in place, you can choose a registered NDIS Support Coordinator or provider who suits your needs. Reach out to them to express your interest and arrange an initial meeting to begin the engagement process.

Initial Meeting with Your Support Coordinator

During the initial meeting, you’ll get to know your Support Coordinator and discuss how they can support you. This meeting sets the foundation for your working relationship and gives you a chance to ask questions, share expectations, and understand their role.

Share Your Plan, Goals, and Support Needs

To help your Support Coordinator understand you better, you’ll share your NDIS plan, including your goals, preferences, current supports, and any challenges. This information helps them create a tailored approach to assist you effectively.

Choose Your Support Providers

With your Coordinator’s guidance, you’ll explore available service providers and choose those who best align with your needs and goals. Your Coordinator can help you compare options, understand service agreements, and ensure a smooth onboarding process.

Monitor Progress Toward Goals

Your Support Coordinator will regularly check in to ensure that your providers are delivering quality services and that you’re making progress toward your NDIS goals. They’ll advocate for your needs and help address any issues that may arise along the way.

NDIS Review Meeting

As your plan review approaches, your Support Coordinator will assist in preparing for your review meeting. They’ll help gather reports, review progress, identify new goals, and support any changes needed to ensure your next plan continues to meet your needs.

What is the difference between Level 2 and Specialist Support Coordination?

How do I choose a Support Coordinator?

You can choose any registered NDIS provider offering Support Coordination services. It’s important to find someone who understands your needs, communicates clearly, and has local knowledge of services in Canberra.

Do I have to stay with the same Support Coordinator?

No. If you’re not satisfied or your needs change, you can switch to another Support Coordinator. Just make sure to follow the terms in your service agreement and inform the NDIA if needed.

Can Support Coordinators help during NDIS reviews?

Yes! Your Support Coordinator can help you prepare for plan reviews by gathering progress reports, reviewing current goals, and identifying new supports or changes needed.

Is Support Coordination the same as Case Management?

Not exactly. While both roles involve coordinating care, Support Coordination is specifically designed to build your capacity and empower you to manage your own supports under the NDIS.
